Eligibility for CTAP Equipment

CTAP offers specialized telephone equipment and network services at no cost to qualified California residents who have difficulty using the telephone. To be eligible to receive equipment through our program, the person must:
  • Live in California
  • Have telephone service at your residence, but it does not need to be in your name
  • Be certified as having one or more of the following disabilities either temporarily or permanently:
    • Deaf/Deafened
    • Hard of Hearing
    • Cognitive
    • Mobility
    • Blind/Low Vision
    • Speech

Cost and Ownership of CTAP Equipment

The California Telephone Access Program (CTAP) is an equipment loan program and the equipment is on loan to you at no cost until you can no longer benefit from it. If the equipment becomes damaged we will replace it. If you plan on moving out of California, contact the CTAP Call Center to arrange for us to pick up the loaned equipment at our expense.

How to Apply - Application Forms

To receive CTAP equipment (one set per household) or CTAP network service provided by this program, you must complete an Application Form. It has two parts:

  • Part A is filled out and signed by or in behalf of the person who will use the equipment.
  • Part B is filled out and signed by an authorized CTAP Certifying Agent.
Certifying Agents Authorized by legislation and/or the CPUC are:
  • CA Licensed Physicians (MD) - can sign for people with any disability.
  • CA Licensed Clinical Audiologist - may only sign for people who are deaf or hard of hearing.
  • CA Licensed Hearing Aid Dispenser - must have the customer record on file, and may only sign for people who are deaf or hard of hearing.
  • CA Licensed Optometrist - can sign only for people who are blind or partially-sighted.
  • CA Department of Rehabilitation Counselor (VRC, RCB or RCD) - can sign for people with any disability.
  • Superintendent or school audiologist at the California School for the Deaf in Riverside & Fremont - may only sign for present students who are deaf or hard of hearing.
  • Veterans Administration Physician - may sign for people with any disability.

Call center hours of operation are: Monday – Friday 7 am to 9 pm, Saturday 9 am to 5 pm, Closed on Sundays.

Application Forms are also available at any one of the seven CTAP Service Centers. They are located in Burbank, Fresno, Oakland, Riverside, San Diego, Santa Ana and Sacramento. Click here for maps and directions.

Once you have an application form:

  • Completely fill out Section A of the form and sign it.
  • Take the form to an Authorized Certifying Agent who will fill out Section B and sign it.
  • When the application form is complete:
Take it to one of the seven CTAP Service Centers. Click here for CTAP Service Center locations. Note: A relative or friend may also bring the application form to the CTAP Service Center for a customer.

Once the application form is approved, we will send a CTAP Call Me Letter requesting that you contact us at our CTAP Call Center. A customer's needs are then determined and matched with the appropriate equipment and services. If needed, the customer can be trained in the use of the equipment and services. Sometimes a combination of equipment and services are needed together to enable the customer to use the telephone. Also, if you are unable to come to a CTAP Service Center, a Field Advisor can arrange a visit to your home to assist you.
